To Report Food or Waterborne Disease or Other Consumer Complaint: A foodborne outbreak is defined as the occurrence of two or more cases of a similar illness resulting from the ingestion of a common food. A single case of suspected botulism or chemical poisoning is also considered as an outbreak of foodborne illness. When we receive a report(s) of a foodborne illness we conduct an investigation primarily to:

1. Identify the agent causing the illness;
2. Identify cases that are part of the outbreak;
3. Determine the cause or error(s) that occurred in the handling of the implicated foods;
4. Prevent further transmission of the agent from current cases, their contacts, or from the
    original source;
5. Prevent future occurrences based on learning experiences from the investigation.

Please report any suspected food borne illness to this department or to the Public Health Department (on menu bar at left) as soon as possible.

To file a complaint about a processed food/beverage, you can print and submit the Consumer Complaint Form.
